Consent Form / Terms & Conditions

The following are the conditions of enrolment to STUDIO 74 LTD

• I agree to pay the terms fees in advance and understand fees are not returnable. Fees are paid termly and any absence due to sickness or prior engagements will not be refunded. Interest can be charged on overdue accounts and teachers have the right to refuse any pupil to attend class if the fees are outstanding.

• I agree to give half a terms notice before leaving. If notice is not given the terms fees will be payable.

• PARKING – I agree to take into consideration the neighbours of Studio 74 and park considerately and to not block driveways or cause obstruction.

• The principal has the right to increase or alter fees as and when necessary

• I agree to complete the registration, medical and photo consent forms

• Students may be asked to leave the school in cases of unreasonable behaviour

• I consent to the teacher being “hands on” in class. Teaching dance will from time to time involve physical contact in order to help children get in the right stance.

• Children should wear the correct uniform / appropriate clothes and footwear.

• I consent to photographs / videos being taken of my child both alone and in a group

• I consent to photos of my child being displayed publicly (newspaper, internet)

• I consent to plasters being put on my child if necessary

• I consent to a doctor / ambulance being called to my child in an emergency

• I understand STUDIO 74 LTD is not responsible for money or articles of value lost or damaged on the premises

• If it is necessary to cancel classes on account of national crisis, extreme weather conditions, epidemic or any other circumstances that the principal has no control over, fees cannot be returned or compensation made for any period of training lost.

However STUDIO 74 LTD will do its utmost to offer alternative classes or arrangements.


Publicity Consent Form

This is to confirm I am happy to have video and photographs taken of my child during performances and presentations during their time at STUDIO 74. This includes other parents/ friends being allowed to take photographs during shows and performances.


Studio 74 Ltd will adhere to the following Rules and Guidelines with regards to Photography and Filming:

1. Images of Children posted on www.studio74dance.co.uk will not be identified by name

2. To reduce the risk of inappropriate use of images only images deemed suitable will be used.

3. Anyone with any complaints about images posted on the website can speak to the principle and the images will be removed.

4. Anyone concerned about inappropriate use of images can report the matter to the Principle and procedures will be followed to report the matter to the appropriate authority.

5. The Expectations for parents or spectators who wish to take photographs or film at an event are to avoid inappropriate or intrusive photography. Any child or parent who has concerns regarding inappropriate or intrusive photography must report their concerns to the Principle who will act in the same manner as they would with any other child protection concern.

6. Studio 74 may use some suitable photographs for newsletters, promotional purposes and for use on the TV screen in our coffee bar area to show what we do in our classes

ONLINE CLASSES: TERMS & CONDITIONS

 

Due to the prevalence of COVID-19 and under guidance from the UK Government STUDIO 74 are delivering lessons via an online video conferencing software called Zoom.

In order to take part in these online lessons, you agree to the following:

  1. Your child will not be left unsupervised at any point in the session.
  2. You can provide at least 6ft2 of clear, flat floor space for the student’s use. Where no furniture, slip and/or trip hazards can be found.
  3. You ensure the student is wearing appropriate clothing and footwear for online classes.
  4. You fully understand that as Studio 74 Ltd are not providing these services in person, we are unable to provide any first aid or safeguarding services.
  5. You understand that Studio 74 Ltd are in no way affiliated with the video conferencing software, and are not responsible for any changes, data loss or software/hardware malfunction as a result of using the equipment. You also agree to their separate terms of use.
  6. You will not hold Studio 74 Ltd responsible for any injury sustained as part of these classes and will watch your child for signs of sickness, tiredness, injury or illness or other incapacitations which may prevent them from taking part.

If you or your child do take part in Zoom / online lessons then you are agreeing to our terms listed above.


With accepting to the terms and conditions I agree the following:

 I agree that the information give herewith is true and correct

 Any personal information is held in accordance with the data protection act

 You have disclosed any medical conditions or allergies to Studio 74 Ltd

 Studio 74 will not be held liable for any injury resulting from the participation in any of the class’s Studio 74 dance has to offer
